Description of a pharmacist managed anticoagulation clinic on 30-day readmission rates after anticoagulant initiation

Background/Purpose: Patients newly initiated on anticoagulation therapy, specifically warfarin, are at an increased risk of adverse events due to the complexity of drug/food interactions and weekly dose finding within a narrow therapeutic window. Studies have shown that the likelihood of bleeding associated with anticoagulation is highest within the first month of outpatient therapy. Our hospital recently opened its first pharmacist-directed anticoagulation clinic. The clinic predominantly monitors warfarin therapy through a Collaborative Care Agreement. This study aims to describe the impact that a pharmacist-managed anticoagulation clinic has on 30-day readmission rates and characterize adverse events in patients newly initiated on anticoagulation therapy.

Methodology: This retrospective chart review has been approved by the local Institutional Review Board. The study will include all patients 18 years of age or older who were newly initiated on anticoagulation therapy from September 1, 2016 through January 31, 2018. The following data will be collected utilizing the electronic medical record system: patient age, gender, weight/height, indication for anticoagulation, liver function, renal function, and comorbidities. The primary endpoint of the study will be 30-day readmission rates with secondary endpoints including time in therapeutic range and adverse events within 30 days of therapy initiation.
